Who are we?
Welcome to the website for the Patient Panel of the Health Centre in Audley
near Stoke-on-Trent in Staffordshire.
The object of the Patient Panel is to be a voice for all patients of the medical practice based at the
Audley Health Centre. We are a group of patients working with the practice to:
contribute to the continuous improvement of service;
foster improved communication between the practice and its patients;
help patients to take more responsibility for their health; and
provide practical support and help to implement change.
The Audley Patient Panel is not a forum for
complaints. If you have a complaint, then you should take this up directly with Audley Health Centre
2015 Patient Survey Results and Action Plan
We’d like to say thank you to the 450 patients who took time to complete the 2015 Patient Survey questionnaire.
Your input is proving extremely useful to the practice in improving the services provided for patients.
The Patient Panel assisted the practice in conducting the survey and in the development of the survey forms and online results recording.
The results were analysed and discussed at practice and patient panel level and then jointly at a patient panel meeting with practice staff. At this meeting an action plan was developed in the format ‘you said....we will’.

Book your appointments online
You can now use the internet to book and cancel your appointments with available doctors/nurses. The service is called Vision Online.
You will need to register with the practice to access this service. Once registered you will be given information that will enable you to create your username and password.
When your account has been created and activated you will be able to book appointments with available doctors/nurses, view existing appointments and cancel appointments if required.
To apply for this service, you need to complete a registration form and return this to the Health Centre. A letter will then be prepared for you to collect from Reception, giving details of how to register. You can view and print the registration form here.
